Are sampled colors exact?

Yes. Clicking the image reads one pixel from the decoded source at its original dimensions. The eight-color palette is an approximation made from a smaller copy for speed.

What happens to transparency?

Transparent pixels keep their alpha value. HEX uses eight digits when needed, and RGB/HSL use their alpha forms. Nearly transparent pixels are excluded from the automatic palette.

Does the image leave this browser?

No. The image is decoded in browser memory and is not sent to a processing server. Exported CSS and JSON contain color values and dimensions, not the source image.
